ECO Supportive Living provides Medicaid waiver home-based services, private pay care, and veteran-focused support to clients across multiple Nebraska counties. Since 2021, we've built our reputation on compassionate, compliant, person-centered care, and our Client Care Managers are at the heart of it.

About the Role

As a Client Care Manager, you'll own a caseload of clients receiving home-based services and serve as the primary link between clients, families, caregivers, and our office team. This is a field-based role: you'll be out in the community conducting home visits, supervising care delivery, and making sure every client's plan of care is followed and documented.

What You'll Do
  • Manage an assigned caseload of clients, including regular home visits and care plan reviews
  • Supervise and support caregivers in the field, including coaching and performance feedback
  • Coordinate with service coordinators, families, and Nebraska DHHS as needed
  • Maintain accurate, timely documentation in our care management system
  • Monitor compliance with Medicaid waiver requirements, client rights, and agency policies
  • Participate in caregiver onboarding, orientation, and ongoing training
  • Respond to client and caregiver concerns with urgency and professionalism
What We're Looking For
  • Bachelor's degree in social work, human services, healthcare administration, nursing, or a related field required
  • Experience in home care, case management, social services, or a related field (Medicaid waiver experience a strong plus)
  • Strong organizational and documentation habits; compliance matters here
  • Excellent communication skills with clients, families, and field staff
  • Comfort working independently in a field-based role
  • Valid driver's license, reliable transportation, and ability to travel within the service area
  • Ability to pass required background checks; CPR/First Aid certification (or willingness to obtain)
