: 8:00 AM – 5:00 PM
Remote with member facing within county (3 days home visits - 2 days work from home)
We’re looking for a dedicated Long-term Service Care Manager to join our team. In this role, you’ll manage a caseload of members with long-term care needs, complete comprehensive assessments, and coordinate health services to ensure seamless, person-centered care.
Key Responsibilities- Conduct monthly and quarterly member contact, including in-person visits (80% travel)
- Perform in-depth member assessments and document care notes
- Collaborate with caregivers, providers, and discharge planners
- Monitor services, follow up with members, and coordinate referrals
- Assist members with complaints and appeals
- Bachelor's degree and 2–4 years of related experience
- 2+ years in Care Management (LTSS, Home Health, or Discharge Planning)
- Familiarity with electronic medical health records and Microsoft Office
- Valid driver’s license (Travel Required)
