What You’ll Do
As a Family Support Coordinator, you will be the ultimate bridge to vital resources, community connection, and advocacy :
Advocate & Empower: Connect families to state and community resources while maximizing their control over their own services .
Guide Transitions: Help individuals smoothly plan and navigate the massive milestone of transitioning from school to adult services .
Navigate Legal & Financial Pathways: Assist with budget administration and facilitate important processes like Guardianship and Power of Attorney .
Collaborate & Document: Facilitate team planning meetings and maintain meticulous documentation alongside a passionate, friendly team .
What We’re Looking For
We need a flexible, creative, and non-judgmental professional who genuinely loves serving diverse families .
- Education: A Bachelor’s degree is highly preferred, but extensive case management experience will absolutely be considered instead .
- Experience: At least 2 years in human services, with a minimum of 1 year in family support . Experience with developmental disabilities is a huge plus!
- Skills: Excellent communication, strong organization, intermediate MS Office proficiency, and a willingness to learn new programs .
- Travel: Reliable transportation for regional/statewide home visits and meetings .
