FACETS is seeking a passionate and mission-driven Outreach Case Manager to join our Region 2 Outreach Program, based out of Bailey's Community Shelter. This dynamic role brings critical, person-centered services directly to individuals living unsheltered in woods, campsites, parks, abandoned buildings, bus stops, and other outdoor environments. Outreach staff actively connect with people experiencing homelessness to build trust, reduce barriers, and link them to essential services.
The Outreach Case Manager plays a key role in helping clients move from the streets into stable, permanent housing, often within 30 days. This is an exciting opportunity to make a direct, meaningful impact on the lives of some of Fairfax County’s most vulnerable residents.
FACETS is a non-profit organization that opens doors by helping parents, their children, and individuals who suffer the effects of poverty in Fairfax County. We meet their emergency needs through our wrap around services and community partnerships and help individuals and families experience safe and stable housing after years of uncertainty and instability. We strive to ensure that every person in our community has a place they can call home.
