Independence Blue Cross is seeking a Care Transition Specialist to support members moving from inpatient care to home or other settings. You will engage post-discharge, educate on medications, and coordinate follow-up with providers to reduce readmissions.
Work is fully remote within the Tri-State Area (Delaware, New Jersey, Pennsylvania). You will collaborate with nurses, social workers, and the care team to guide members through plan benefits and resources while documenting Outreach outcomes
