Northern Rivers Family of Services is seeking a Clinical Time Transition Program (CTTP) Youth Peer Advocate in Schenectady, NY. The role centers on supporting youth transitioning from hospital to community, leveraging lived experience to promote reintegration and stability.
You will collaborate with youth, families, schools, and community resources, while reporting to the H2H Program Director. Some evenings or weekends and onsite travel are required, with potential remote work options.
