* Essential Duties and Responsibilities:

*** Maintains an ongoing minimum caseload of 25 families. This requires outreach and recruitment of families on an ongoing basis.
* Completes an assessment with each participant upon enrollment. This assessment identifies participants’ strengths, resources, goals, barriers and needs.
* Develops Individual Service Plans and works one-on-one with participants to promote participation and engagement, setting goals in the areas of employment, education and training, financial management, and personal/ family well-being.
* Establishes and maintains cooperative relationships with area human service agencies, facilitates appropriate referrals to specialized support services and resources that address household needs including but not limited to child care, substance abuse, behavioral health, youth services, credit counseling, financial literacy, GED, employment training, career development and services to support seniors.
* Assists participants with their education and training interests, including identifying suitable education and training opportunities.
* Reviews service plan goals and performs necessary follow-up to ensure clients are progressing towards established goals.
* Manages household crisis situations and monitors household status.
* Conducts resource briefings and employment/ education events.
* Maintains accurate and current participant files.
* Other duties as assigned.

* Education and Experience:

** Bachelor’s degree in human or social services, public administration, or a related field required. Four (4) years of human development case management experience to include working with low-income individuals and families.  
** Knowledge and Skills Requirement:
*** Demonstrated knowledge of the Family Self-Sufficiency Program and the characteristics, needs and interests of housing choice voucher and public housing residents.
* Ability to manage an active case load of up to 150 participants effectively and efficiently.
* Strong ability to work with individuals with complex issues and apply accepted case management techniques including appropriate assessment, goal setting, referral, monitoring and limited crisis management.
* Ability to apply client centered interviewing techniques.
* Ability to motivate and guide participants through the stages of change leading to new behaviors and habits.
* Demonstrated characteristics of initiative and resourcefulness, tact, courtesy and sensitivity to the needs of participants.
* Ability to initiate and coordinate service delivery and follow-up services.
* Experience communicating and working successfully with individuals from a broad range of socio-economic and cultural backgrounds.
* Ability to prepare, present and communicate ideas in a clear, concise and effective manner, both orally and in writing.
* Ability to work as a member of a team.
* Ability to establish and maintain effective and courteous working relationships with other employees, residents, and community agencies.
* Must have strong knowledge of human development resources and community organizations with the ability to manage third party relationships on behalf of participants.
* Demonstrated ability to work independently, fully execute assignments, receive instruction, and work without intense supervision.
* Demonstrated ability to use time-management skills to achieve success with schedules, workloads and deadlines. Travel within Atlanta city limits and home visits are required.
* Ability to develop, create, and deliver Human Development presentations internally and externally.
* Commitment to the participants, the program and its stated goals.

* Working Conditions:

** Works in an office environment. Travel within Atlanta city limits, home visits of HCVP households, and program site visits are required. May require some bending, stooping, reaching and prolonged sitting. May require some lifting usually not exceeding 10 lbs. Must have and maintain a valid state of Georgia driver’s license and be fully insurable for the purpose of obtaining clearance on company “Authorized Driver List.”
