Upperline Health Plusis seeking aPrimary Care Medical Assistant who offershighquality,patient centered care in avalue-based practice.

In our patient-centered and outcome driven environment the Medical Assistant supports patients in office with dutiesin-office such as rooming patients, obtaining medical history and taking vital signsas well as ensuring care beyond the moment bycoordinatingcare, supporting extended care teameffortsandconductingproactive outreach to improve outcomes and patient experience. The MA also provides direct support to the Upperline Plus Primary Care Provider to ensure smooth clinical operations and effective team-basedcare.

This position will be on-site in our Ontario, CA office.

• Room patients, obtain and document vital signs, medical history, and chief complaints
• Assist NP with health assessments and inclinicvisits with Upperline Plus patients being seen inourclinics
• Facilitate and support virtual provider encounters from the clinic setting
• Accurately document clinical information in the electronic health record (EHR)
• Performs thorough andaccuratemedication reconciliation at each patient encounter by reviewing, verifying, andconsolidatingall prescription medications, over-the-counter drugs, supplements, and herbal products
• Identifymedicationdiscrepancies or potential concerns; andensuresthe reconciled medication list is complete, current, and accurately documented in the electronic medical record for provider review
• Telephonic outreach and engagement with Upperline Plus patients to coordinate ongoing preventative care
• Manage in-basket messages, prescription refill requests, and prior authorizations
• Care coordination activities as assigned including, but not limited to, medical records requests, order management, appointment scheduling,managing patient care referrals,DMEreferrals, etc.
• Identifying community resources and assisting
