Kingston Health Sciences Centre (KHSC) is seeking two experienced Nurse Practitioners to step into a pivotal Most Responsible Provider (MRP) role within our Patient Transitions & Clinical Resources portfolio
, supporting care within the Alternate Health Facility – Transitional Care Unit

This is an exceptional opportunity for an autonomous, systems‑focused NP who thrives in complex environments and is passionate about patient flow, safe transitions, and continuity of care. As the NP‑MRP, you will hold full clinical accountability for patients across their episode of care—leading assessment, diagnosis, treatment, discharge planning, and follow‑up within your legislated scope of practice.

About the Role

As a Nurse Practitioner – MRP
, you will function as the primary clinical provider
, coordinating and managing care to ensure timely, high‑quality transitions while optimizing patient outcomes and system efficiency.

Working within an advanced nursing practice framework, you will collaborate closely with physicians and interprofessional partners, consulting or transferring care when needs extend beyond NP scope. Beyond direct clinical care, this role offers meaningful opportunities to influence practice through leadership, education, research, and quality improvement initiatives aligned with KHSC’s strategic priorities.

You will also serve as a clinical leader and mentor
, supporting staff, students, and teams while championing innovation, professional excellence, and patient‑and family‑centred care.

Key Responsibilities Clinical Practice
  • Deliver comprehensive, autonomous care including advanced assessment, diagnosis, treatment, prescribing, procedures, and discharge planning.
  • Hold full accountability as the Most Responsible Provider
    , ensuring safe, timely, and effective care across transitions.
  • Lead patient flow planning and proactively prevent delays by collaborating with the interprofessional team and leadership.
  • Serve as the primary point of contact for patients, families, and care teams to ensure continuity and coordinated follow‑up.
  • Arrange transfer of care to physicians when patient needs exceed NP scope of practice.
Leadership & Professional Practice
  • Act as a clinical leader and expert resource, promoting excellence in nursing practice.
  • Contribute to the development, implementation, and evaluation of policies, standards, and clinical practice guidelines.
  • Participate on clinical and organizational committees and system‑level initiatives.
  • Identify clinical risks and support mitigation through collaboration and evidence‑informed decision‑making.
  • Advocate for patients, families, and populations in care delivery, health systems, and policy contexts.
  • Support learning and professional development of nurses and interprofessional team members.
Research & Quality Improvement
  • Integrate research and best‑practice evidence into clinical decision‑making and program innovation.
  • Participate in quality improvement and research initiatives to enhance patient outcomes and advance nursing practice.
  • Evaluate care processes and identify gaps and opportunities for system improvement and future research.
Education
  • Serve as a clinical educator, mentor, and preceptor to advanced practice learners and staff.
  • Support academic partnerships and contribute to NP student education and preceptorship.
  • Design and deliver education using adult learning principles.
  • Provide consultation and education to community and system partners to support collaborative practice.
Required Qualifications
  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Adult, Pediatric, Neonatal, or Primary Health Care).
  • Master of Science in Nursing or equivalent.
  • Minimum three (3) years of recent clinical nursing experience.
  • Previous experience as an MRP Nurse Practitioner (preferred).
