Hospice Admissions Registered Nurse / RN

Mission Healthcare, a leading home health and hospice provider in the western United States, is hiring for a Registered Nurse to join our hospice admissions team. This role involves initiating and revising plans of care, conducting assessments, developing and implementing palliative care plans, and serving as case manager when assigned.

12:00pm – 10:30pm (Rotating days and weekends)

• Initiates the plan of care and makes necessary revisions as patient status changes.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan that establishes goals, incorporates palliative nursing actions, and includes patient and family in the planning process.
• Provides health‑care instructions to the patient as appropriate per assessment and plan.
• Assists the patient with activities of daily living and facilitates efforts toward self‑sufficiency and comfort care.
• Acts as case manager when assigned by the clinical manager and coordinates patient care for assigned caseload.
• Communicates with the physician regarding patient needs, reports changes in condition, and obtains physician orders as required.
• Teach patient and family/caregiver self‑care techniques, medication, diet, and other instructions, and utilizes opportunities for health counseling with patients and families.
• Works in concert with the interdisciplinary team.

• Graduate from an accredited school of nursing; bachelor’s degree preferred.
• Current state license as a Registered Nurse.
• Minimum 1 year of recent acute care experience in an institutional setting.
• Minimum 1 year of recent experience in home care, preferably hospice.
• Valid driver’s license and reliable, insured transportation.

$55.00 – $63.00 hourly (depending on experience)
