Cruz Home Health Care LLC is seeking a compassionate, skilled Registered Nurse to join our dedicated home health team in Ontario, CA. Responsibilities:- Conduct comprehensive patient assessments and develop individualized care plans for home health patients
- Administer medications, wound care, IV therapy, and other skilled nursing treatments in the home setting
- Monitor and evaluate patient health status, documenting changes and reporting to physicians as needed
- Educate patients and family members on disease management, medication adherence, and home safety
- Coordinate care with interdisciplinary team members including physical therapists, social workers, and physicians
- Maintain accurate and timely clinical documentation in compliance with Medicare, Medicaid, and agency standards
- Ensure patient care aligns with OASIS assessments and home health plan of care requirements
- Active and unrestricted Registered Nurse (RN) license in the state of California
- Minimum 1 year of clinical nursing experience; home health experience preferred
- Knowledge of OASIS documentation, Medicare/Medicaid home health regulations a plus
- Valid California driver's license, reliable transportation, and current auto insurance
- Strong clinical assessment, critical thinking, and time management skills
- Excellent communication and patient education abilities
- Current CPR/BLS certification required
- Willing to travel to Los Angeles, San Bernardino, and Riverside Counties
