Overview
Gentiva is seeking a Registered Nurse (RN) hospice and palliative care provider to support patients and families in the community across Ontario, Ohio.
Key Responsibilities- Serve as primary point of contact for patients and families, guiding them through their hospice journey.
- Deliver and document skilled, hands‑on nursing care based on individualized plans of care in patients' homes or assisted living settings.
- Conduct initial and ongoing assessments to monitor patient condition and adjust care plans as needed.
- Collaborate with interdisciplinary team members—including physicians, LPNs, CNAs, social workers, chaplains—to refine care plans.
- Provide emotional, physical, and spiritual support to patients and loved ones, including education, comfort, and bereavement guidance.
- Teach caregivers how to safely provide care, offering practical training and reassurance.
- Communicate important updates on patient condition to the care team to ensure timely interventions.
- Maintain accurate, up‑to‑date clinical documentation and adhere to infection control and compliance standards.
- Support quality improvement and scheduling initiatives to maintain consistent, compassionate, efficient care.
- Assist patients and families in understanding medication administration, hospice philosophy, symptom and pain management, and end‑of‑life processes.

- Education: Registered Nurse license required.
- Experience: Minimum of one (1) year as an RN in acute care, oncology, long‑term care, hospice, or home health. Hospice or palliative care experience preferred.
- Licenses, Certifications and Registration: Current nursing license in state of employment; CPR certification; valid driver’s license and automobile insurance.
- Specialized Knowledge and Skills: Knowledge of terminally ill patients and hospice services; understanding of hospice roles; excellent patient assessment skills; knowledge of community resources; strong oral and written communication; documentation and program management; knowledge of general nursing practice; understanding of managed care principles and regulatory guidelines.
Patient homes and/or institutional settings; automobile use; potential exposure to infections, communicable diseases, hazardous materials.

- Tact and professionalism in handling crisis situations.
- Flexibility to perform varied tasks efficiently and with composure.

Proficiency with nursing assessment tools, computers, fax, copier, and mobile devices; reliable transportation required.
