Description

This position is established to provide service coordination to the Agency's properties, either owned or contracted with, as well as to individuals residing in their own homes. The Service Coordinator is responsible for managing and assisting individuals in building support networks, conducting assessments, and developing service plans to help them accomplish their goals, Additionally, the Service Coordinator serves as a liaison between property management, residents, family members, service agencies, case manager's, and community resources to ensure accessibility and foster strategic partnerships.

Requirements
  • Primary Responsibilities:
  • Assist individuals in their homes, community, or clinic settings and communicates the purposes of program and available supports to participants.
  • Conducts assessments to identify socio-economic factors affecting overall health and wellbeing.
  • Collaborates with participants to develop individualized health and social management plans with measurable goals.
  • Coordinates intake activities, visit and assessment schedules, and ensures accurate and timely documentation of all required reports and billing.
  • Supports individuals in accessing community and healthcare resources, including scheduling appointments, completing applications, making referrals, and advocating as needed.
  • Facilitates communication and coordination between healthcare providers, social service agencies, and family support.
  • Maintains and continually expands knowledge of community resources, available services, programs, and best practices in public interactions and service coordination.
  • Builds and maintains trusting, collaborative relationships with program participants, community partners, and team members.
  • Promotes individual wellness and coordinates educational programs for residents, families, and staff to increase awareness of community resources.
  • Assists residents in developing informal support networks with other residents, family, and friends.
  • Coordinates programs with service organizations to provide onsite and/or offsite services for residents.
  • Maintains and updates a directory of community resources.
  • Networks with service providers to maintain existing relationships and explore additional service opportunities through private, local, state, and federal partners.
  • Distributes newsletters, flyers, brochures, or other informational or educational materials to residents and program participants.
  • Documents service plans, activities, and outcomes in accordance with Agency policies and funder requirements.
  • Ensures all work complies with regulatory and funder guidelines, including accurate billing and reporting.
  • Participates in data collection, reporting and evaluation of program outcomes.
QUALIFICATIONS
  • Associates degree and 2 years work experience preferred.
  • Ability to obtain certification within the first year of employment along with becoming a member to the AASC (American Association of Service Coordinators).
  • Strong verbal and written communication skills.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
  • Strong decision making and critical thinking abilities.
  • Ability to analyze information, evaluate needs and implement effective solutions.
  • Excellent organization, time management, and accuracy in a flexible work environment.
  • Ability to establish and maintain effective relationships with internal staff, external partners, and the public.
  • Ability to work independently and collaboratively as part of a multidisciplinary team.
  • Ability to communicate professionally, respectfully, and effectively.
