Requirements:
Successful completion of NP or PA Graduate Program (Masters Level), AANP or ANCC certification for Nurse Practitioners, NCCPA certification for Physician Assistants, Ability to obtain:
Idaho and Oregon medical licensure, and valid controlled substance registration with the Idaho and Oregon Board of Pharmacy and DEA, 2 years' experience as a Sleep Medicine Provider preferred
