Position: LICENSED PRACTICAL NURSE

Ohio Health is seeking a dedicated LPN to join our Healthcare & Medical Services team. In this role, you will provide compassionate, patient-centered care including medication administration, basic treatments, patient education, and coordination with RNs and providers. You will document accurately in the EMR, support care plans, and contribute to a safe, inclusive environment for all patients and families. Responsibilities

  • Provide direct, hands-on nursing care to patients under the supervision of RNs and physicians
  • Obtain and document vital signs, patient histories, and routine assessments within LPN scope of practice
  • Administer medications and treatments according to provider orders and Ohio
  • Health policies
  • Assist with wound care, specimen collection, and basic diagnostic procedures
  • Prepare patients and exam rooms for examinations, procedures, and treatments
  • Support providers and RNs during exams, minor procedures, and emergencies
  • Monitor patient status and promptly report changes in condition to the RN or provider
  • Document all care, interventions, and patient responses accurately in the electronic medical record
  • Reinforce patient and family education on diagnoses, medications, and discharge instructions
  • Follow infection prevention, safety, and regulatory standards at all times
  • Collaborate with the interdisciplinary care team to support individualized care plans and safe discharges
  • Participate in quality, safety, and performance improvement initiatives within the unit or department

Required Skills

  • Vital signs measurement and basic patient assessment

  • Electronic medical record (EMR) documentation, preferably Epic

  • Time management and prioritization in a clinical setting
  • Clinical triage support for common symptoms within LPN scope
  • Culturally competent and trauma-informed patient care
