Position Summary:

The Stepping Up Case Manager provides support to individuals with serious mental illness (SMI), substance use disorders (SUD), or co‑occurring conditions as they transition from incarceration back into the community. • Monitor client progress post‑release, adjusting service plans and supports as needed to reduce recidivism and promote recovery.
  • Maintain timely, accurate, and confidential documentation in accordance with agency and regulatory standards.
