Licensed Practical Nurse (LPN)Are you a compassionate Licensed Practical Nurse (LPN) looking for a rewarding career where your skills are valued and your work truly matters? We'd love to have you join our team!
As an LPN, you'll provide exceptional nursing care to residents in a supportive long-term care and rehabilitation environment. Working alongside Registered Nurses, physicians, therapists, and other healthcare professionals, you'll help develop and implement individualized care plans, support residents on their rehabilitation journey, and ensure they receive the highest quality of care with dignity, respect, and compassion.
Responsibilities- Administer medications and treatments as prescribed by physicians.
- Monitor and document residents’ health status, including vital signs, and report changes to the supervising RN or physician.
- Assist with activities of daily living (ADLs), including bathing, dressing, and feeding, as needed.
- Collaborate with the rehabilitation team to support residents’ recovery goals, including physical, occupational, and speech therapy plans.
- Maintain accurate and timely documentation in accordance with facility policies and state/federal regulations.
- Provide emotional support and education to residents and their families.
- Ensure a safe and clean environment, adhering to infection control protocols.
- Current and valid LPN license
- Strong clinical skills, including medication administration and wound care.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team-oriented environment.
- Compassionate, patient-focused approach to care.
- BLS/CPR certification required.
