Job SummaryJob Summary: Oversees wound care within the facility to ensure that all residents are free of avoidable pressure ulcers and that residents with pressure ulcers receive treatment and services to promote continued improvement and resident safety. Manages the infection control program of the facility.
Essential Functions And Responsibilities- Review all new admissions/re-admissions to determine resident’s risk of developing pressure ulcers and to implement interventions to reduce the risk. If pressure ulcers are present, the Wound Care nurse is responsible to make certain that interventions are in place to promote healing and reduce the risk of complications.
- Complete the Pressure Ulcer Risk Evaluation when indicated.
- Maintain weekly documentation regarding the size, stage, depth, odor, drainage and progress, utilizing the Weekly Pressure Ulcer Record and the Weekly Wound Assessment.
- Monitors progress of wounds and ensures that physician order changes are obtained as indicated to promote healing.
- Communicate with family/responsible person regarding progress/status of wound.
- Review all pressure ulcers at the weekly NAR meeting and maintain documentation.
- Attend monthly CQI meeting and present statistics/trends relating to infection control and wound care.
- Complete necessary documentation for all community and nosocomial infections.
- Compile and maintain monthly statistics of all infections.
- Participate in the weekly Comprehensive Meeting.
- Assist in providing staff with information/in-service education regarding infection control and wound care as indicated by facility needs.
- Participate in facility staffing on a designated schedule as needed.
- Participate in and complete all required in-service educational programs and competency evaluations.
- Assist with monthly review of Physician orders.
- Assure resident safety at all times.
- Other duties as assigned.

- Employee must be able to perform the essential functions of the position with or without reasonable accommodations. Physical requirements may include frequent use of hands to finger, handle or feel objects, tools or controls; reach with hands and arms; talk or hear. The employee may be required to lift, stand, walk, sit, climb or balance, stoop, kneel, crouch or crawl and smell.
Vision abilities may require close vision, distant vision, color vision, peripheral vision, depth perception and the ability to adjust focus. - For the safety and security of residents and employees, must be able to read, write, and verbally communicate in English.
- Must be able to lift and/or move up to 30 pounds, and occasionally up to 50 pounds.
- Must be capable of maintaining regular attendance.
- Must meet all local health regulations and post offer physical exam.
- Must maintain a professional, respectful, friendly demeanor with all residents, family members, guests, and staff at all times.
