Halifax Health Hospice in Orange City, FL is seeking a Licensed Practical Nurse to provide direct patient care to hospice patients across settings under RN supervision. Day/Night/Weekend/Holiday shifts, 24/7 coverage as assigned.
Responsibilities include patient assessment, education for patients and families, documentation in EMR, and participation in the Interdisciplinary Plan of Care and QA activities. LPN license in Florida and BLS required; hospice experience preferred.
