Smart Phone or Laptop Official College Transcripts Two Letters of Recommendation (From a colleague, former employer, or acquaintance | cannot come from family members or friends) Don't have a letter? No problem—your references can complete a form online. I-9 Documents (Two physical forms of ) Business casual dress code No jeans Fingerprinting Please complete the Vendor Application Form prior to orientation so we can verify your fingerprinting requirements.

• Renewal Fee: $47.77
