Mental Health Professional II PRN

HCA Florida Orange Park Hospital is seeking a Mental Health Professional II PRN to deliver patient‑focused mental health services to patients and their families. The role involves assessment, psycho‑education, therapy facilitation, and care coordination within a collaborative multidisciplinary team.

• Conduct one‑on‑one, group, and family psycho educational and process groups.
• Develop treatment plans and attend ongoing treatment planning meetings and reviews.
• Assess environment and patient safety status and respond to safety issues or changes in the patient's condition.
• Identify risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
• Complete assessments, group notes, and all other documentation promptly and in compliance with requirements.
• Collaborate with the multidisciplinary team to assess and coordinate patients’ discharge planning needs and readiness.
• Conduct post‑discharge follow‑up as indicated.

• Basic Cardiac Life Support certification within 30 days of employment start date.
• Nonviolent Crisis Intervention certification within 90 days of employment start date.
• Master’s Degree in a related field.
• Licensure: LPC/LMHC, LCSW, LMFT, LPC‑A/MHCI, LBSW, or LMSW/RCSW‑I (or equivalent).
• One year of relevant experience required.

No travel required.
