Friday evenings 5pm to 11:30pm, Saturday and Sunday 8am to 11:30pm

On-Call RNs play a crucial role in end‑of‑life care, stepping in during off‑hours to bring relief, clarity, and compassion when it is needed most. • Collaborate with Remote Triage On Call Team to deliver hands‑on care to hospice patients and offer supportive services to families during after‑hours (if applicable).
• Deliver hands‑on care to hospice patients and offer supportive services to families during after‑hours (if no Remote Triage team).
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medication or adjust care based on hospice protocols or provider orders.
• Communicate with the physician regarding changes in condition and obtain orders as necessary.
• Offer guidance and support to the patient and family in understanding the current condition and anticipating next steps.
• If a death occurs during on‑call hours, pronounce death, provide support to the family, contact the funeral home, and notify the hospice team.
• Complete timely documentation and communication.

• Graduate of an accredited nursing school with current licensure to practice in the state of operation.
• At least one year of nursing experience required; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an insured vehicle meeting state or organization requirements and in good working order.
