• Administer skilled nursing care for clients of all ages in their place of residence.
• Assess home health and/or hospice patients in stable and emergent situations.
• Implement, develop, document, and evaluate the Plan of Care (POC).
• Educate patients and/or their families regarding care, diet, medications, and home safety.
• Supervise and provide clinical directions to healthcare assistants.
• Monitor assigned cases to ensure compliance with third-party payer requirements.
• Provide effective communication and serve as a liaison to patients, families, and team members.

Shift: M-F until shift is complete;
On-call required if requested.

Patient Service Area:
Local travel required; reliable transportation needed.

• Current state license as a Registered Nurse.
• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
• CHPN certification preferred.
• Proof of current CPR certification.
