Job SummaryThe Day Habilitation/Community Habilitation R Provider serves as a role model and advocate; ensures individuals’ safety by providing consistent supervision and observation and promotes a nurturing learning environment in the Day Habilitation Without Walls, the Community and on-site in Dominical Hall. The Day Habilitation/Community R Provider implements individuals’ Comm Hab R Plans and provides training of adult living skills in a dignified and respectful manner.
Duties & ResponsibilitiesThe Day Habilitation and Community Habilitation R Provider ensures individuals safety by providing consistent supervision and observation. The Day Habilitation and Community Habilitation Provider assists in the development, implementation and documentation of Individual Habilitation Plans, Life Plans and Day/Community Habilitation Plans. They perform tasks on-site and in the community that involves training individuals who have a developmental disability in various life skill areas, that promote fuller personal development, enjoyment and expanded independence, community integration, productivity and individualization.
The Day Habilitation and Community Habilitation R Provider must successfully complete AMAP Certification and administer medications as prescribed by a doctor(s) to the individuals as soon as scheduled, within 90 days of hire and must successfully complete Tube feeding training, Diabetic Training, Colostomy training, and any specialized certification, if required, within 90 days of hire.
Education & Experience- High School Diploma or GED required; some college in Human Services or related field preferred.
- Experience working with individuals with developmental disabilities strongly preferred.
- Valid driver’s license and reliable transportation required.
- Must be awake during the entire shift. Sleeping on the job is prohibited.
- While performing the duties of this job, the employee is regularly required to communicate clearly and effectively with staff and individuals.
- All SCIP-R physical interventions (Perform Strategies for Crisis Intervention and Prevention as necessary for physical restraint and control of individuals exhibiting aggressive behaviors including one-person escort, standing wrap, seated wrap, and one-person take down onto side). Must be able to travel stairs.
- SPECIAL DEMANDS:
Must be able to do a one-person transfer. - When necessary and use lifts provided for all lifting. They also need to be able to lift a minimum of 25 pounds. On occasion it may be necessary to lift a greater weight, in which case the employee needs to be able to identify whether or not to ask for assistance.
