The RN travels to a patient's home to administer services and helps patients maintain their independence.

Duties include:

administering at-home IVs, changing dressings, cleaning wounds, in-depth assessments, overseeing patient treatment plans, and updating doctors and the agency on the patient’s health. Documentation and communication are done through Synergy Software.

Patients need care 7 days a week. You are required to work at least two—three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer).

City close to you with a group of patients who need care for that day.

• Providing nurse visits to home health patients (home visits).
• Performing initial, comprehensive assessments. Document and upload to Agency Portal.
• Obtaining and documenting physician orders.
• Initiating and regularly re-evaluating and revising the plan of care in consultation with the Clinical Team Manager.
• Coordinating care with the Clinical Team Manager, physician and other members of the home care team; informing them of significant changes in the patient’s condition and needs.
• Performing re-certifications, resumption of care, transfers, and discharges as requested; completing and submitting all related documentation.
• Assessing the need for services of other team members (PT, OT, ST, MSW, HHA).
• Performing Home Health Aide supervisory visits per regulatory requirements.
• Demonstrating knowledge of regulatory requirements.

• 1 year visiting RN for Home Health (required).
• 1 year nursing experience (required).
• Current CA RN license (required).
• BLS from the American Heart Association and/or the American Red Cross (required).
• NSO nursing license insurance (required).
• IV/Phlebotomy certified (required).
