We are looking for a clinically experienced nurse who has nursing documentation/charting skills and is a fast learner with great communication skills.
RoleThe RN travels to a patient's home to administer services and helps patients maintain their independence. Duties include administering at-home IVs, changing dressings, cleaning wounds, in-depth assessments, overseeing patient treatment plans, updating doctors and the agency about patients' health. Documentation and communication with the agency is done through Synergy Software.
AvailabilityPatients need care 7 days a week. Flexibility in scheduling is needed, but you must be able to work at least two to three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer).
Responsibilities- Providing nurse visits to home health patients (home visits)
- Performing an initial, comprehensive assessment and documenting everything & uploading to the agency portal
- Obtaining and documenting physician orders
- Initiating and regularly re-evaluating and revising the plan of care in consultation with the assigned Clinical Team Manager
- Coordinating care with the assigned Clinical Team Manager, physician, and other members of the home care team, informing them of significant changes in the patient's condition and needs
- Performing re-certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager; completing and submitting all related documentation
- Assessing the need for the services of other team members (PT, OT, ST, MSW, and HHA)
- Performing Home Health Aide supervisory visits per regulatory requirements
- Demonstrating knowledge of regulatory requirements
- Visiting RN for Home Health: 1 year (Required)
- Nursing: 1 year (Required)
- Current CA RN License (Required)
- BLS from the American Heart Association and/or the American Red Cross (Required)
- NSO Nursing License Insurance (Required)
- IV/Phlebotomy Certified (Required)
