HCBA Registered Nurse

Overview

Access TLC is seeking a HCBA Registered Nurse (RN) to join our HCBA department. The HCBA Waiver provides care management services to persons at risk for nursing home or institutional placement. The care‑management team coordinates Waiver and State‑Plan services and arranges for other available long‑term services and supports in the local community.

The HCBA RN is responsible for assessing, care planning, assisting with locating, coordinating, and monitoring services for community‑based participants on the waiver. In‑person visits will be in the Los Angeles areas.

This position is full‑time. Initial assessment/reassessment visits are paid $150 per visit and Level 3 follow‑up visits / Plan of Treatment visits are paid $50 per visit. Mileage is reimbursed.

• Registered nurse with current licensure to practice professional nursing in the State of California.
• Minimum of one year nursing experience.
• Excellent observation, verbal and written communication skills.
• Current CPR certification.
• Valid California Driver’s License with proof of a good Motor Vehicle Record.
• Current proof of auto insurance with state minimum (or greater) coverage.
