Description:
Shift days are Monday to Friday.
Shift hours start at 08:00 and vary based on clinic hours.
The contract length is 6 months.
Responsibilities include coordinating care for patients with various gastrointestinal conditions, including Crohn's disease and inflammatory bowel disease.
The role involves working closely with physicians and assisting in patient education and follow-up.
Daily tasks include managing patient messages and calls, performing minor injections, and handling refill requests.
The nurse navigator will conduct patient discharges, providing education and follow-up instructions related to medications.
Communication with off-site labs for lab results and follow-ups will be required.
The nurse navigator will also handle authorization processes and prioritize care based on medical needs.
This position involves collaboration with interdisciplinary teams and managing patient information through designated systems.
The nurse navigator should have strong organizational skills and be able to work in a fast-paced environment.
