Role OverviewWe’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across our partner communities. This role offers steady, predictable patient volume and the opportunity to deliver high-quality care directly where seniors live.
What You’ll Do- Routine & urgent assessments; chronic disease management
- Post-discharge follow-ups, med reconciliation, labs/diagnostics
- Coordinate with facility staff, specialists, families/POAs
- Same-day documentation; CMS/regulatory compliance
Full-time; weekday schedule preferred. Routes are clustered by region to minimize travel.
Requirements- Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
- Geriatrics or facility-based experience preferred; EMR proficiency
Annual compensation range: $140,000-$170,000 (based on experience, schedule, and patient volume)
Additional earning opportunities available based on coverage and panel needs
W-2 with benefits available; mileage reimbursed per policy
