Position Summary:
In collaboration with the Stroke Program Manager, the Stroke Coordinator assists in the planning, implementation, coordination, and continuous evaluation of the hospital's Stroke Program to ensure the delivery of high-quality, evidence-based patient care. This role supports the achievement and maintenance of national stroke certification standards by coordinating clinical, operational, and quality improvement initiatives across the continuum of stroke care.
The Stroke Coordinator partners with the Stroke Program Manager and collaborates with physicians, advanced practice providers, nursing staff, emergency medical services (EMS), emergency department personnel, radiology, laboratory, rehabilitation, and case management to promote timely stroke recognition, treatment, and recovery. The coordinator facilitates communication among multidisciplinary team members to improve care coordination, ensure adherence to evidence-based guidelines, and optimize patient outcomes.
Responsibilities include supporting stroke certification readiness and regulatory survey preparation; coordinating staff education, reviewing stroke cases to identify opportunities for quality improvement; monitoring compliance with established stroke protocols and performance measures; and providing ongoing feedback to clinical teams to improve patient outcomes. The Stroke Coordinator also serves as a clinical resource for staff, patients, families, and community partners by promoting stroke prevention, education, and community awareness.
In partnership with the Stroke Program Manager, the coordinator supports strategic initiatives that strengthen the organization's stroke program, foster interdisciplinary collaboration, and advance excellence in stroke care.
Required Qualifications
- Understanding of performance improvement processes
- Strong verbal and written communication skills
- Strong analytical skills with the ability to interpret data
- Must demonstrate professionalism and sensitivity to diverse cultural backgrounds at all times and in all situations
- Must demonstrate excellent organizational and multi-tasking skills
- Must demonstrate customer service skills appropriate to the job
- Minimum 5 years of experience caring for stroke patients in the Acute Care hospital setting.
- Good organization skills and ability to prioritize assignments
- Demonstrated experience working with physicians, nurses, and others to effectively develop and initiate patient care plans
- Computer skills, especially Microsoft Office, Word, Excel
- Ability to maintain a work pace appropriate to the workload
- Ability to establish and maintain effective working relationships across the Health System
- Familiar with University of California policies and procedures
