The W Assisted Living Communities is seeking a compassionate and dedicated Case Manager. We are looking for a candidate with a proven track record of successfully coordinating services for residents while remaining committed to excellence in resident satisfaction. The selected candidate will work with a positive and supportive team of healthcare professionals.

• Maintain firm adherence to the resident’s “Bill of Rights”, as they relate to the facility
• Communicate with the resident’s care providers
• Assessments and coordination of admission/re‑admission from the community, nursing homes and hospitals
• Coordinate and supervise clinical care for residents
• Maintain clear and concise documentation in accordance with facility requirements

• Prior experience in Long Term Care is a plus or 1 year of experience as a Case Manager required
• Previous Compuware experience is a plus
• Excellent phone skills and ability to work with families/other care providers
• Strong organizational skills, passion and compassion for the population
• Ability to work cooperatively with multidisciplinary team
