Position:
Limited Term Case Manager (OPI and OPI-M/OPI-C programs)

Program:
Oregon Project Independence (OPI) and OPI-M/OPI-C; supports older adults and people with physical disabilities. Includes essential services to promote quality of life, independent living, and access to home- and community-based services. Allowable services include personal care, home care, chore services, assisted transportation, adult day care, respite, case management, nursing, and home-delivered meals. The Case Manager assists with planning, eligibility, service coordination, and ongoing monitoring.

This is a Limited Term position with anticipated duration up to three (3) years.

Responsibilities
  • Assist OPI-C clients with long-term care planning, determine program eligibility, develop person-centered service plans, advocate for and coordinate services, and provide ongoing monitoring and oversight of the service plan.
  • For OPI-M eligible individuals, complete annual service plans after eligibility determination; assist with planning, coordination, advocacy, and monitoring to ensure stability and sustainability of service plans.
  • Conduct client interviews, assess needs, risks, strengths, and eligibility; develop, implement, and monitor service or case plans; evaluate progress and adjust as needed.
  • Coordinate with community resources, benefits, and service providers; assist with applications and required documentation; advocate to ensure access to services.
  • Explain program requirements, monitor eligibility, identify barriers, and develop solutions; maintain confidentiality and comply with regulations.
  • Prepare reports, case summaries, and required program records; utilize case management systems and participate in meetings and trainings.
Qualifications

Required Minimum Qualifications / Transferrable Skills

  • Minimum of two (2) years' direct service experience providing case management to older adults and adults with disabilities.
  • Minimum of two (2) years' experience in assessment and evaluation of needs, including effective communication, record-keeping, and confidentiality; ability to develop care plans.
  • Minimum of two (2) years' experience with interviewing, counseling, crisis triage, case planning for diverse needs and settings; ability to establish cooperative relationships with diverse groups; capacity to manage an ongoing caseload.
  • Ability to relate to a wide variety of people from diverse social, economic, and cultural backgrounds.

Preferred Qualifications / Transferrable Skills

  • Bilingual in Spanish, Russian, or SE Asian language.
  • Professional experience with older adults or Alzheimer's/dementias.
  • Experience entering information into statewide databases such as RTZ or Oregon Access.
Pre-Employment Requirements
  • Pass post-offer, pre-employment drug test; review County policy.
  • Criminal history check (may include fingerprinting).
  • Must drive for County business; valid driver’s license and acceptable driving record required.
  • Veterans’ Preference: list relevant transferable skills in the application if applicable.
