Position: Case Manager (Child & Family Treatment Team)

Case Manager

Clackamas County Health Centers is seeking a collaborative Case Manager to join the Child & Family Treatment Team at the Lake Road Health Center in Milwaukie. This position provides case management, care coordination and skill-building services for children, youth, young adults and their families who are navigating mental health, substance use and/or co-occurring needs. In this role, you will work directly with clients and families to identify needs, strengthen skills, connect with community resources and services, and support progress toward individualized treatment goals.

Services may be provided in a variety of settings, including the clinic, clients' homes and community-based locations. The Case Manager works closely with behavioral health clinicians, psychiatric providers, primary care providers, schools, social service agencies and other community partners to help clients and families access coordinated and responsive care.

We are looking for a compassionate and resourceful professional who is passionate about supporting children, youth, young adults and families as they navigate behavioral health needs and complex service systems. The ideal candidate will be skilled at building trusting relationships, navigating complex service systems, responding effectively during challenging situations, and balancing independent work with collaboration as part of a multidisciplinary treatment team.

Required

Minimum Qualifications / Transferrable

Skills:

  • Qualified Mental Health Associate (QMHA) as established by Oregon Administrative Rule (OAR) or the Mental Health & Addiction Certification Board of Oregon (MHACBO).
  • Experience providing case management, care coordination and/or skill-building services to children, youth, young adults and families.
  • Must possess and maintain a Basic Life Support (BLS) certificate.

Preferred Special

Qualifications:

  • Experience working as part of a multidisciplinary behavioral health or healthcare treatment team.
  • Experience working with individuals experiencing mental health, substance use and/or co-occurring disorders.
  • Experience supporting clients (youth, children and transition aged youth (18-24), experiencing emerging psychosis and/or substance use disorders.

Pre-Employment Requirements:

  • Must pass post-offer, pre-employment drug test.
  • Must pass a criminal history check which may include national or state fingerprint records check
  • Driving is required for County business on a regular basis or to accomplish work. Incumbents must possess a valid driver's license, and possess and maintain an acceptable driving record throughout the course of employment.

Typical Tasks:

  • Provide case management and care coordination services to children, youth, young adults and families experiencing mental health, substance use and/or co-occurring needs.
  • Identify and coordinate resources, referrals and supports; monitor client progress; and assist clients and families in accessing services that support treatment goals, stability and overall well-being.
  • Provide individual and family skill-building services based on identified treatment needs and service plans.
  • Assess client and family needs through interviews and ongoing engagement with clients, parents, caregivers, guardians, providers and other involved individuals.
  • Coordinate services with multidisciplinary treatment partners, including behavioral health clinicians, psychiatric prescribers, primary care providers, schools, social service agencies, insurance providers and other community partners.
  • Facilitate communication and service coordination to promote continuity of care and successful transitions between services.
  • Engage clients and families in service planning and ongoing care.
  • Support participation in assessment and treatment planning, provide outreach when clients miss appointments or need additional engagement, and help clients and families identify and work toward individualized goals.
  • Provide advocacy and system navigation for clients and families with healthcare providers, schools, social service agencies, public and private organizations, the justice system and other community partners.
  • Assist with problem-solving, accessing resources and addressing barriers to services.
  • Respond to client needs and crises in the most appropriate setting, which may include the clinic, home, community or virtual environment.
  • Work collaboratively with clients, families and treatment team members to support safety, stabilization and connection to appropriate services.
  • Maintain accurate and timely electronic health records and documentation in accordance with agency policies and procedures, Oregon Administrative Rules, insurance requirements and applicable Medicaid or Medicare requirements.
  • Develop, update and maintain required service plans, eligibility information and other case documentation.
