Serve as a link between the clinic setting and the community to facilitate access to services and improve the quality and cultural competence of services delivery. Assist by increasing health knowledge of patients and communities through a broad range of activities such as transportation to health visits, outreach, community education, informal counseling, social support, and advocacy.
Requirements: Requires both a high school diploma or equivalent and a valid Oregon Driver's License with a clean driving record. Certified Nursing Assistant (CNA) certification is preferred, or 2 years of verifiable healthcare experience. Must submit to and pass a criminal background check with Federal, Tribal, and State Law Enforcement agencies. Also, a pre-employment alcohol and drug screening is required. Must be insurable through the tribe's insurance program to operate Tribal or GSA vehicles.
