** Clatsop Behavioral Healthcare (CBH) has moved most positions to a 4 day 9 hour work week with paid 30 minute lunches to promote a better work life balance. Wages have also been increased for our valued current and future employees.
** Clatsop Behavioral Healthcare has an immediate opening for a full time Service Coordinator for our Developmental Disabilities (DD) program. The primary purpose of this position is to advocate for individuals in Developmental Disability services and make sure that they are receiving the most suitable services available. Work with a diverse population and with very specific support needs and identify individual’s strengths and challenges to provide support in a safe manner.
Conduct quarterly monitoring of foster homes and group homes of all support being provided within the home.
** Essential Functions/Major Responsibilities**:

* Develop ISPs (Individual Support Plans) and all related documentation. This includes, but not limited to:
Risk Identification Tracking forms, Person-Centered Information Forms, and Related support documents
* Conduct and/or participate in needs assessments (Oregon Needs Assessments, Child Needs Assessments, Adult Needs Assessments, Support Needs Assessment Profile) All assessments are conducted and/or reviewed by the Services Coordinator
* Provide Choice Counseling to ensure all available choices/options are explored. Assist to coordinate chosen services
* Monitor services, client records, and documentation in group homes, foster homes, and/or supported individuals’ own homes to verify standards are being met, based on the Oregon Administrative Rules, including Medication Administration Record (MAR) Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IADLs), finances, safety, support plans being followed, implementation of Behavior Support Plans, Incident Reports, Protocols, Fire Evacuation Records, Behavior Data, etc.
* Facilitate team meetings for each client at least annually and as needed
* Complete Case Management Contacts as dictated in the ISP
* Document all client contacts/maintain current, accurate PNs (Progress Notes) and bill for services
* Identify housing for individuals seeking residential services outside of their family home. This may include children and adult foster homes, group homes, out-of-county placement and supported living. Services are then developed on client desires, needs and interests
* Complete annual Level of Care for K-Plan and Waiver services
* Complete Plan of Care in eXPRS to reflect and approve funding
* Approve Personal Support Worker timesheets in eXPRS
* Complete Notice of Planned Action and assist individual or family to file hearing request if desired
* Attend required meetings and trainings
* Advocate for clients’ rights and services.
* Collaborate with community providers.
* Obtain a minimum of 20 hours per year DHS sponsored or other training in the area of developmental disabilities to remain in compliance with OAR (Oregon Administrative Rule) requirements
* All CBH Staff are Mandatory Abuse Reporters per the State of Oregon, and as such all staff must be aware of and follow Mandatory Abuse Reporting policies.
* Ensure client confidentiality is maintained and adheres to all ethical and professional standards

** Education/Experience Requirements:
*** Knowledge of the public service system for Developmental Disability services in Oregon.
* Bachelor’s degree in Behavioral Science, Social Science, or a closely related field; or a Bachelor’s Degree in any field AND one year of human services related experience; or an Associate’s Degree in Behavioral Science, Social Science, or a closely related field AND two years human services related experience;…
