Position: Care Coordinator (Veterans)
Location: Madras

Description

The Veterans Care Coordinator/Navigator will provide care coordination/patient navigation services to Jefferson County veterans who are engaged in behavioral health services at the Jefferson County Community Mental Health Program (CMHP) – CCBHC/Certified Community Behavioral Health Clinic.

Duties
  • Veterans Care Coordinator/Navigator duties will largely consist of the following (not be limited to):
  • Direct interactions with veterans who are open in behavioral services at the Jefferson County CMHP-CCBHC.
  • Telephone contacts, which include veterans in CMHP-CCBHC services, who need support, may be struggling and may need crisis mental health services and/or rapid access to SUD assessment, detox, and treatment services. Also, doing reminder calls to veterans who are scheduled for assessments, groups, individual sessions, medication management appointments, and other Best Care services in Jefferson County.
  • Transportation: taking clients to some counseling/treatment appointments, when appropriate and if approved by supervisor.
  • Helping veterans in behavioral health services connect with medical services, including primary care, and helping them connect with OHP Assisters when indicated.
  • Referrals to and assistance with accessing various services, when appropriate (i.e. Food Bank, LINC, primary care/medical services, community college, parenting, etc.). Case management services, including documentation.
  • Helping clients develop and connect to community-based pro-social or wellness-oriented recovery activities and veterans’ activities.
  • Being part of a CCBHC-CMHP behavioral health treatment team and working closely with clinicians, medical practitioners, and peer support.
  • Connecting with the CCBHC-CMHP Prevention-Health Education Team to work on veterans’ suicide prevention strategies, initiatives, and community trainings.
  • Other duties as assigned by the Program Director or Mental Health-Adult Team Program Manager.

  • Works independently and collaboratively as a positive member of the Best Care team;
  • Performs other organizational duties as assigned.
Requirements

QUALIFICATIONS:

Education And/Or Experience Requirements
  • 1. High School diploma or equivalent; bachelor’s degree or 5 years of relevant experience in the behavioral health field.
  • 2. Familiarity with veterans/services as well as mental illness and substance abusing populations.
  • 3. If in recovery from mental illness and/or substance abuse, must be able to demonstrate continuous sobriety under non-residential, independent living conditions for the immediate past two years.
Licenses And Certifications
  • CADC-I or QMHA-I certification required or

Eligibility to register within 1 week of hiring to obtain QMHA-I within 6 months or CADC-I within 1 year

  • Must maintain a valid Oregon Driver License or ability to obtain one upon hire and be insurable under the organization’s auto liability coverage policy (minimum 21 years of age and with no Type A violations in the past 3 years, or three (3) or more Type B violations) and must have a reliable vehicle with adequate insurance to travel as needed.
Preferred
  • Associate’s degree in a behavioral health field
  • Bilingual in English/Spanish a plus
