• $20.50 – $23.25/hour depending on experience and skills (+$1.00/hour increase at 120 days)

Oakridge is an area in Oregon with high healthcare needs. Orchid Health’s Oakridge Clinic is located near the Cascade Mountains, offering access to recreational opportunities nearby.

The Community Health Worker is responsible for:

• Helping patients and families navigate community services and resources (SDOH) in a welcoming and safe environment
• Assisting OHP patients to engage in care and maintain insurance coverage
• Supporting providers and the Care Management Team in delivering whole-person care through CCM and community outreach
• Acting as a local liaison and patient advocate within the community

• Establish trusting relationships with patients and families; provide general support and create a welcoming environment
• Conduct initial outreach and engage patients in care plans focused on Social Determinants of Health (SDOH) with ongoing follow-up
• Provide ongoing follow-up, basic motivational interviewing, and goal setting around SDOH needs
• Participate in an integrated team to support chronic care management and behavioral health needs
• Coordinate services and develop care plans as an active care team member
• Conduct outreach for newly assigned patients and those losing insurance to help stay covered and establish care
• Be knowledgeable about community resources relevant to patient needs
• Provide resources and referrals to community agencies and ensure successful connections
• Build and maintain relationships with community organizations and partners
• Engage with the community and act as a community organizer
• Follow-up on CHW referrals and document all patient contact in the EHR (within 24 hours)
• Exhibit positive relations with patients, visitors, staff, and partners to fulfill Orchid’s mission
• Attend regular staff meetings and trainings

• Education:
  
  Minimum high school diploma or GED; higher education preferred
• Understanding of rural communities served; preference given to lived experience
• One year of relevant experience with underserved populations in a social service or community-based setting
• Basic computer skills; preference for EHR experience
