SALARY: Step 38 ($87,240) – 50 ($124,384)

CLASSIFICATION: Professional, Regular, Full-Time

LOCATION: Klamath Tribal Health & Family Services

Wellness Center, Chiloquin, Oregon

This Registered Dietitian position primary responsibility is to assist with the nutritional needs of the patients seen at Klamath Tribal Health & Family Services (KTHFS). Routine duties will include providing dietary patient care and nutritional education in the clinic, at other KTHFS service location, and at alternate job sites. Other duties will include providing support services for tribal programs, nutrition program development, educating staff, and community outreach.

1. Conduct nutritional assessment and implement nutritional patient care plans consistent with current guidelines.

2. Adapt care plans with patients to be consistent with lifestyle, cultural, psychosocial, economic, and physiological factors.

3. Document patient visits in the Electronic Health Record (EHR) in accordance with KTHFS procedures.

4. Provide direct patient care Conduct nutritional assessment and implement nutritional patient care plans in collaboration with the multidisciplinary team.

5. Develop and maintain a KTHFS Nutrition Services Program Protocol and Procedures for common disease states treated with Medical Nutrition Therapy (MNT), dietary support of existing KTHFS programs, and community outreach events.

6. Provide MNT, develop and maintain dietary care protocols of common disease states (diabetes, chronic kidney disease, cirrhosis, hypertension, chronic heart failure, cholesterol disorders, malnutrition, malabsorption disorders, etc) that will include the guidelines followed, educational material to use, patient case-management process, scheduling process, and reporting productivity & clinical measure.

7. Develop dietary protocols for support of KTHFS and Tribal programs (medical, dental, pharmacy, and health education departments; tribal meal sites; commodities program; etc) will include the type of dietary support, educational material & guidelines used, frequency of interaction, process for other programs to request dietary support and reporting on productivity & other measure.

8. Develop dietary protocols for community outreach at KTHFS, tribal, and local events (Diabetes & Diabetes Prevention Program walks/events;
Restoration; elder’s, women’s, men’s, and children’s events; education booths/set-up in KTHFS lobbies; etc) will include type/frequency of events, educational material used, the process for requesting dietitian involvement in events, and reporting on productivity & other measures.

9. Assist with food insecurity programs and operations that increase access to fresh produce including cooking demonstrations and reoccurring food pantry.

10. Review, evaluate & select or prepare suitable nutritional education materials for patient and staff education within KTHFS departments and programs.

11. Effectively teach fundamental classes and any additional pertinent material individually, in family settings, and in a classroom environment. Ability to have flexible schedule to maximize participation will be needed. Prepare meals and/or snacks for program participants, as needed.

12. Prepare and submit reports that will include productivity and outcomes measure for direct patient care, nutritional program development, KTHFS program support, grant program support, and community outreach activities in compliance with the Privacy Act, HIPAA and KTHFS policies.
