• Plan and conduct segments of treatment programs designed to restore and improve physical, social and mental functions, meeting department objectives.
• Serve as co-leader in an interdisciplinary approach to patient groups.
• Assess patients, develop and implement individualized treatment plans, and monitor progress.
• Document progress and coordinate with the care team to optimize outcomes.
• Provide patient and family education on home exercise programs and safety.

• Active physical therapy license in Oregon
• Experience in outpatient physical therapy (outpatient setting preferred)
• Ability to work effectively in a hospital-based outpatient setting
• Strong communication and collaboration skills
• Comfortable working as part of an interdisciplinary team
