Travel LPN/LVN – LTC/SNF | Blue Mountain Care Center – Prairie City, OR

Duration: 13 weeks
Shift: Nights, 12-hour shifts
Guaranteed Hours: 36 hrs/week
Unit: LTC/SNF – Nursing Administration
Care Setting: Post-Acute
Charting: Paper

• Minimum 1 year experience
• Active LPN/LVN license (required to submit)
• BLS Certification
• BSN not required

Provides nursing care per Oregon Nurse Practice Act standards in a long-term care setting. Supervises CNA staff, oversees medication administration and treatments, participates in admissions/discharges, coordinates care with physicians and interdisciplinary teams, manages resident condition changes, ensures regulatory compliance, and supports survey readiness. Acts as patient advocate and maintains a safe care environment.

Facility is a Critical Access Hospital (CAH).
