The primary purpose of the LPN position is to provide direct nursing care to the residents and to supervise the day‑to‑day nursing activities performed by nursing assistants, in accordance with federal, state, and local regulations.
Essential Duties Administrative Functions- Direct the day‑to‑day functions of the nursing assistants in accordance with governing rules and regulations.
- Ensure compliance with the written policies and procedures established by the facility.
- Review policies, procedure manuals, and job descriptions; recommend revisions.
- Plan the shift’s services, programs, and activities with nursing staff.
- Maintain a current Nursing Service Procedures Manual.
- Ensure personnel comply with procedures in the manual.
- Make written and oral reports concerning shift activities as required.
- Coordinate nursing services with other resident services for total care.
- Confirm personnel compliance with job descriptions.
- Participate in the development and implementation of the quality assurance program.
- Assist in facility surveys and inspections by government agencies.
- Review and update resident discharge plans as needed.
- Assist in planning the nursing portion of discharge plans.
- Interpret policies and procedures for staff, residents, visitors, and agencies.
- Admit, transfer, and discharge residents as required.
- Complete accident/incident reports as necessary.
- Write resident charge slips and forward to the Business Office.
- Maintain the Daily Census Report and submit to the Business Office.
- Complete administrative duties such as medical forms, reports, evaluations, studies, charting.
- Maintain confidentiality of user and passwords; report violations.
- Protect resident protected health information; report violations.
- Report unauthorized attempts to access the facility’s information system.
- Complete and file required record‑keeping forms upon admission, transfer, or discharge.
- Encourage attending physicians to review treatment plans and sign orders.
- Record telephone orders and transcribe them to resident charts and medication cards.
- Chart nurses’ notes in an informative and descriptive manner.
- Fill out accident/incident reports and submit to the Director.
- Chart all reports of accidents or incidents involving residents.
- Record new or changed diet orders; forward to Food Services.
- Report all discrepancies with physician orders or charting errors to the Nurse Supervisor.
- Complete transfer forms in accordance with procedures.
- Document unauthorized discharges in the resident’s medical record.
- Perform routine charting duties as required.
- Sign and date all entries in the resident’s medical record.
- Prepare and administer medications as ordered by the physician.
- Verify resident identity before administering medication.
- Ensure proper medication dispensation and avoid cross‑assignment.
- Maintain adequate supply of floor stock medications and supplies.
- Order prescribed medications, supplies, and equipment per policy.
- Keep narcotic records accurate for your shift.
- Notify the Nurse Supervisor of all drug and narcotic discrepancies.
- Review medication cards for completeness and accuracy.
- Notify the attending physician of automatic stop orders before last dosage.
- Dispose of drugs and narcotics as required.
- Participate in employee performance evaluations and staffing recommendations.
- Inform the Nurse Supervisor of staffing needs when personnel fail to report.
