Licensed Practical Nurse (LPN) at Anchor Post Acute, a 120-bed skilled nursing and rehab facility located behind Aiken Mall, Aiken, South Carolina. The role involves providing direct nursing care and supervising Certified Nursing Assistants on 12-hour night shifts (7:00 PM–7:00 AM), with weekend shifts available.
Compensation & Benefits- Hourly pay $30–$33.
- Shift differentials: $4 on weekend day shifts, $6 on weekdays, $7 on weekend nights.

General purpose: provide direct nursing care and supervise day-to-day nursing activities performed by nursing assistants in compliance with federal, state, and facility guidelines.
Essential duties: include, but are not limited to the following categories:
Administrative Functions- Direct day-to-day functions of nursing assistants.
- Ensure all nursing personnel comply with written policies.
- Review and recommend updates to policies and procedures.
- Plan shift services, programs, and activities with staff.
- Maintain current Nursing Service Procedures Manual.
- Report on shift activities as required.
- Coordinate with other resident services.
- Ensure compliance with job descriptions and quality assurance program.
- Participate in facility inspections and surveys.
- Review and update resident discharge plans.
- Admit, transfer, and discharge residents as necessary.
- Complete accident/incident reports, write resident charge slips.
- Maintain daily census report and perform other administrative duties.
- Guard data confidentiality and report violations.
- Report unauthorized attempts to access facility information.
- Complete and file required record-keeping forms.
- Encourage physicians to review and sign treatment plans.
- Record and transcribe orders into resident charts.
- Chart nurses’ notes accurately.
- Submit accident/incident reports to Director.
- Record new or changed diet orders and report discrepancies.
- Complete transfer forms and unauthorized discharge documentation.
- Perform routine charting duties.
- Sign and date all entries in the resident’s medical record.
- Prepare and administer medications ordered by physician.
- Verify resident identity prior to administration.
- Ensure medications are administered correctly.
- Maintain adequate floor stock and report needs.
- Order medications, supplies, equipment as required.
- Maintain narcotic records accurately.
- Notify Nurse Supervisor of discrepancies.
- Review medication cards for completeness and adherence to policies.
- Notify attending physician of automatic stop orders.
- Dispose of drugs and narcotics as required.
- Participate in employee evaluations and staffing decisions.
- Notify Nurse Supervisor of staffing needs and absentee calls.
- Review and evaluate workforce and recommend changes.
- Develop work assignments and provide leadership.
- Conduct daily rounds and report issues.
- Ensure all assistants are certified or trained.
- Assist in correcting problem areas and improving services.
- Maintain SOPs and inter-departmental rapport.
- Ensure staff adhere to safety and infection control procedures.
- Maintain proper computer workstation security.
- Report occupational exposures and incidents.
- Report fraud to Administrator.
- Inform staff of new admissions and room assignments.
- Ensure rooms ready for admissions.
- Greet and escort new residents.
- Orient new residents and families.
- Coordinate with physicians for rounds.
- Arrange diagnostic and therapeutic services.
- Consult with physician on resident care.
- Review charts and implement nursing objectives.
- Check administered treatments and evaluate resident status.
- Coordinate with social and activity programs.
- Report accidents/incident to physician.
- Notify physicians and next of kin of changes.
- Handle restorative and rehabilitative programs.
- Inspect nursing service areas for cleanliness.
- Provide catheterization, tube feeding, suction, dressings, colostomy, drainage, labs, bandages, massage, range of motion, care of the deceased, etc.
