Shift: Day shift, 8 hours (08:00 - 17:00)
Schedule: Monday – Friday (flexible); weekends and nights as needed

Licensure/Certification s: Oregon Medical License required
Experience: Prior experience preferred but not mandatory; comfort with pediatrics and adults essential

Pay & Benefits:

Hourly Pay Rate: $0.01/hr (rate details discussed upon application)
Pay Schedule: Weekly via Direct Deposit

What You’ll Do:

Conduct annual risk assessments and reviews for patients with chronic conditions
Provide telehealth services (~70%) and in-home visits (~30%), including pediatric and adult patients
Perform assessments and escalate urgent findings to primary care providers
Manage a schedule of approximately 12-14 telehealth visits or 6-7 home visits per day
Document patient interactions thoroughly and accurately
Travel mileage reimbursed at IRS standard rate
Collaborate with the care team to ensure continuity of care

What You Need:

Active Oregon Medical License
Certification as a Nurse Practitioner
Ability to see both pediatric and adult patients
Experience with telehealth and home health preferred
Strong communication and assessment skills
Ability to work independently and as part of a team
