Full‑time shifts available in Lyons, OR.

Schedule:

Tue‑Thu, 7:30 am to 7:30 pm. The role involves caring for G‑tube, trach, and ventilator dependent patients. Prior experience is preferred.

• Provide skilled nursing care for patients with G‑tube, trach, and ventilator dependence.
• Deliver home‑care services in patients’ homes.
• Maintain accurate electronic documentation using the Aveanna tablet system.
• Follow all health‑related requirements, including vaccinations and testing.

• Active, unencumbered LPN license.
• Current CPR certification with hands‑on component (or ability to obtain upon hire).
• TB skin test current within last 12 months.
• Six months prior hands‑on nursing experience preferred but not required.
• Reliable transportation.
