Heartline Staffing, in partnership with Purple Cow Recruiting, is seeking an experienced Travel Registered Nurse for a Home Health assignment in Medford, Oregon and surrounding areas. This role supports adult patients in a home setting, delivering skilled nursing care through independent visits and care coordination.
Position OverviewThis is a field-based role ideal for nurses comfortable working independently, managing their own schedules, and delivering comprehensive in-home patient care.
Assignment Details- 13-week contract
- Day shift | 5x8s (08:00–16:30)
- No weekends
- No on-call
- Up to 6 visits per day (30 productivity points standard)
- 1:1 patient care model
- Gross: $2,646 per week
- W2: $35.00 per hour
- Tax-Free Stipend: $1,246 per week
- Perform Start of Care, Admissions, Recertifications, and routine visits
- Provide wound care and medication management
- Complete discharge visits
- Maintain infection control standards
- Document using OASIS and EMR systems

Requirements- Active RN license REQUIRED (in hand prior to submission)
- Minimum 1 year Adult Skilled Home Health experience REQUIRED
- OASIS documentation experience REQUIRED (HARD STOP)
- Experience with Start of Care, Admissions, and Recertifications REQUIRED
- BLS certification REQUIRED
- Experience with wound care and medication management REQUIRED
- Computer documentation experience REQUIRED
- Reliable transportation REQUIRED
