Responsibilities
- Communicate regularly with residents and their families about Plans of Care, PT, OT, and other treatment protocols.
- Attend and participate in morning meetings and stand‑ups to facilitate team communication.
- Answer residents’ questions about care, treatment plans, illness progression, and other issues to ensure they feel safe and secure.
- Monitor and adjust resident status based on changing needs and conditions.
- Organize and prioritize daily work by assessing new, current, and discharging resident needs in area(s) of responsibility.
- Complete documentation as required.
- Perform utilization review activities to provide resident appropriate, timely, and cost‑effective care.
- Coordinate care with residents, care providers, facilities financial services, and third‑party payers.
- Oversee all admissions and discharge activities.
- Coordinate referrals both to and from the facility.
- Build positive, effective, and appropriate relationships with residents, families, staff, and professional colleagues.
- Ensure accurate charting and timely completion of necessary paperwork.
- Work independently and exercise sound judgment in interactions with physicians, providers, payers, residents, and families.
- Communicate and promote cooperation and collaboration among residents/families/caretakers, physicians, nurses, and other ancillary partners.
- Demonstrate excellent time‑management skills to develop organized work processes in a high‑volume, rapidly changing environment.
- Utilize intermediate computer skills for documentation and communication.
- Maintain professional, respectful, and honest interactions with residents, families, staff, and partners.
This position has no supervisory responsibilities.
Qualifications- Bachelor’s Degree in Nursing or Social Work.
- Registered Nurse (RN) license preferred;
Licensed (LVN or LPN) nurse acceptable. - Two (2) years of clinical nursing experience preferred.
- Knowledge of Medicare, Medicaid, and related benefits.
- Knowledge of PCC (Case Management) principles.
- Proficient in reading technical procedures and policy manuals.
- Strong mathematical skills: fractions, percentages, ratios, and proportions.
- Strong reasoning and problem‑solving abilities.
- Certificates and licenses: LVN/LPN valid license, RN valid license, and Certified Case Manager (CCM) certificate a plus.
- Must maintain all required continuing education and licensing; remain in good standing with the Department of Public Health.
Standing and/or walking frequently; sitting occasionally; reaching with hands and arms frequently; pushing/pulling frequently; talking and/or hearing frequently; tasting and/or smelling frequently; lifting up to 50 pounds frequently; climbing, balancing, stooping, kneeling, crouching, or crawling occasionally; vision with close, distance, color, peripheral, and depth perception.
