A facility near Crescent City, California, is seeking a full‑time, permanent Registered Nurse Case Manager to join their team.
Pay: $42.00–$51.21/hour
Registered Nurse Case Manager Opportunity- Work in either Home Health or Hospice settings
- Rotate on‑call during holidays
- Collaborate with a multidisciplinary team to develop and implement personalized treatment and discharge plans
- Coordinate services that align with patient needs, payer guidelines, and clinical goals to ensure seamless transitions of care
- Registered Nurse license required
- At least one year of case‑management experience in a clinical setting
