Hospice Registered Nurse (RN) Case Manager– Full-Time, Part-Time, PRN, and On-Call Opportunities

Our team is currently seeking skilled and dedicated Registered Nurses to join our hospice team in
full-time,
part-time
, PRN
, and on-call capacities in the Portland Metro areas.

Position Overview

As a Hospice Registered Nurse, you will play a crucial role in delivering comprehensive, patient-centered care to individuals in their homes, skilled nursing facilities, or assisted living communities. You’ll collaborate with the interdisciplinary team to manage symptoms, provide education, and ensure our patients experience comfort and dignity throughout their hospice journey.

Key Responsibilities
  • Deliver compassionate, skilled nursing care focused on symptom management, pain relief, and emotional support.
  • Conduct comprehensive assessments and develop individualized care plans in collaboration with the interdisciplinary team.
  • Educate patients and families about disease progression, medications, and care options.
  • Provide timely and accurate documentation in accordance with regulatory requirements and organizational policies.
  • Coordinate care with physicians, social workers, chaplains, and other hospice team members.
  • Participate in on-call rotations and PRN assignments as needed to support patient needs.
Qualifications
  • Current Registered Nurse (RN) license in Oregon.
  • Hospice or home health experience required
  • Strong interpersonal skills with a commitment to providing exceptional patient care.
  • Ability to work both independently and collaboratively as part of an interdisciplinary team.
  • Valid driver’s license, reliable transportation, and current auto insurance.
  • Supportive and collaborative work environment.

  • Mileage reimbursement for travel to patient homes.
  • Pay range starting at $52.5 an hour depending on experience and education
Employment Type

  • Part-Time:Regularly scheduled hours each week with consistent patient assignments.
  • PRN (Per Diem):Flexible, as-needed shifts to help cover vacations, high census, or specific patient needs.
  • On-Call: Availability to provide support outside regular business hours for urgent patient needs.
  • Full-Time: Regularly scheduled hours each week with consistent patient assignments.
