Locum Tenens Nurse Practitioner – In-Home Wellness Assessments in Oregon. Join a growing locum tenens Nurse Practitioner opportunity in Oregon providing in-home wellness visits and risk adjustment assessments for Medicare members and other at-risk populations. This on-site role offers meaningful one-on-one patient care in the home setting while supporting quality outcomes, accurate clinical documentation, and value-based care initiatives. Providers will serve assigned counties including Clackamas, Deschutes, Hood River, Jackson, and Marion, while enjoying the natural beauty, vibrant communities, and outdoor lifestyle Oregon is known for.

• Compensation Structure: $120-$130 per assessment.
• Setting:
  In-home wellness visits and risk adjustment assessments.

• Duration: 52 weeks.
• Schedule:
  
  4 shifts per week.
• Shift Duration: 8-hour days.
• Hours:
  
  32 hours per week.
• Employment Type:
  
  PRN / Locum Tenens.
• Coverage Area:
  Clackamas, Deschutes, Hood River, Jackson, and Marion counties in Oregon.
• Travel:
  Regional travel required, with day-to-day responsibilities generally within a 55-mile radius.

• At least 1 year of Nurse Practitioner experience required; 3 years of patient care experience preferred.
• Valid, unencumbered Nurse Practitioner license in Oregon.
• Experience in primary care, adult care, geriatric care, home-based care, or home health assessments preferred.
• Previous in-home risk assessment or Medicare wellness assessment experience preferred.
• Ability to work independently and effectively within an EMR system.
• Reliable transportation to travel to patient homes within an assigned Oregon service area.
• Strong communication, empathy, and relationship-building skills.
• Bilingual skills are a plus.

• Perform annual in-home wellness visits and risk adjustment health assessments for Medicare members and other at-risk populations.
• Complete comprehensive assessments including physical examination, body systems review, vital signs, medication review, functional status review, pain assessment, cognitive screening, fall risk screening, and social assessment.
• Evaluate the home environment and identify factors that may impact patient health and well-being.
• Provide patient education related to medications, pain management, preventive care, and overall wellness.
• Assist in closing quality care gaps, including screenings and labs.
• Document findings thoroughly and communicate relevant clinical information appropriately.
• Build trust with members through compassionate communication and empathy in the home setting.
• Collaborate to support enhanced care delivery, care coordination, and quality outcomes.
• Travel throughout the assigned county and other areas of Oregon as needed.
