About the Role
Remo Medical Group, P.A., the affiliated medical practice of Remo Health ("Remo"), is hiring a remote Advanced Practice Clinician.
Remo is seeking a Psychiatric Mental Health NP (PMHNP) or Adult Gerontological NP (AGPCNP) who is dedicated to the assessment and longitudinal care of patients living with dementia. This role operates within an Interdisciplinary Team framework, requiring specific expertise in dyad care - supporting both the patient and their caregiver - alongside our robust longitudinal care navigation model of care that aligns clinical decisions with What Matters Most to the family.
We are looking for a clinician who thrives in a fast-paced startup environment and is excited about operating at the intersection of medicine and technology. As an Advanced Practice Clinician you will evaluate and manage a panel of patients with cognitive disorders and their caregivers (dyads) as well as be an integral part of the interdisciplinary team which includes Neurologists, Geriatricians, Care Navigators, Registered Nurses, Nurse Practitioners, Licensed Clinical Social Workers, and Pharmacists.
You will report directly to our Medical Director and work closely with them on hiring and training new providers as we expand to new markets. Remo clinicians help to continually define how our organization serves patients, families, and care team members, and work cross-functionally to ensure that our processes and tools are built to support individuals living with dementia and their family caregivers.
What You’ll Be Doing
Clinical Practice & Operations:
Diagnostic Evaluations: Perform comprehensive assessments to establish, confirm and stage dementia diagnoses.
Care Plan Formulation: Drive the creation and implementation of evidence-based Care Plans covering clinical, behavioral, safety and caregiver support goals.
Dementia Proficiency: Stay current with the latest advancements in dementia care, translating and sharing this expertise across the team to ensure the highest quality of clinical care.
Quality Improvement: Proactively identify opportunities to optimize the Care Model and the technical platform, directly enhancing dyad satisfaction, clinical quality and the overall provider experience.
Care Delivery:
Longitudinal Support: Manage a panel of patients living with dementia and their caregivers over time, partnering seamlessly with the Interdisciplinary Team.
Dyad-Centric Care: Evaluate the distinct needs of the patient-caregiver unit, anchoring care interventions around “What Matters Most” to them.
Crisis Prevention: Proactively monitor for cognitive changes and environmental stressors to minimize acute escalations, unnecessary emergency department visits and hospitalizations.
